Toyota Expands Takata Air Bag Safety Recall. FDA expands recall of blood pressure drug due to cancer-causing agent. How often have you heard about product failures lately? It is not a coincidence that many of the root causes of the defect is always blamed on the “supplier.” Take Toyota and the air bag incident. It was a Takata air bag. A supplier! Same with the blood pressure drug. No surprise it too was blamed on a supplier problem! Who are these ‘poor performing’ suppliers? Who is responsible for their poor performance?
